Survey without opening back-end connection

Hi

I have created a survey following the link Survey processing without opening a connection to the CRM Backend from the Internet - CRM - SCN Wiki

However when testing the survey I see that the mail will be sent by Outlook is not containing any details as subject/ mail body.

I  am not sure how the data is being sent to CRM System..

Is there anything I am missing here.
