Lead template in Campaign is not shown, why?

Hi All,

I am trying to generate leads from campaign.  I have followed the below steps for the same.

Now, I have selected the Campaign type as 'Lead Qualification' and Communication method as 'Lead Generation' in Campaign.  But Lead Template field is not filled automatically.

I clicked on Lead Template field to select the transaction type but the below screen is appeared.

Please help me how the Lead Template is determined in Campaign to generate lead.
